INFORMATION ABOUT THE PROCESSING OF PERSONAL DATA

With this responsibility in mind, we process your personal data as the Data Controller in accordance with the Personal Data Protection Law No. 6698 (“Law”) and related legislation.

1. COLLECTION, PROCESSING, AND PURPOSES OF PROCESSING PERSONAL DATA

Hekimoğlu Dental Clinic carries out personal data processing activities in accordance with Article 20 of the Constitution and the principles in Article 4 of the Personal Data Protection Law. The clinic retains personal data for the period required by law or for the purpose for which they are processed. Personal data are collected to provide health services within the legal framework and to fulfill the contractual and legal obligations of the clinic in a complete and proper manner. Personal data may vary depending on the health services provided, and are collected verbally, in writing, or electronically via automated or non-automated methods such as call centers, websites, online services, and similar means.

Your personal data including your name, surname, Turkish Identity Number, passport number or temporary Turkish Identity Number, place and date of birth, marital status, gender, unique patient number specific to our clinic, and other identity data that can identify you; your address, phone number, email address, and other communication data; financial data such as payment and billing information; voice and digital information that may be obtained by us at various times through electronic or non-electronic methods; video recordings if you visit our clinic; vehicle plate data if you use the parking lot; general and special categories of personal data obtained during the provision of all medical diagnosis, examination, treatment, and care services; data related to your private health insurance and Social Security Institution data for the financing and planning of health services, and financial data such as payment and billing information sent through our website will be processed by our clinic.

Furthermore, your personal data may be processed when you use our call center or website to use online services, participate in trainings, seminars, or events organized by the clinic, or visit or browse our websites. Your personal data will be processed under the conditions and purposes specified in Articles 5 and 6 of the Personal Data Protection Law. Your personal data, including sensitive personal data, will be processed by our clinic for the following purposes: Fulfilling our legal obligations in the Health Services Basic Law No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the Private Hospitals Regulation, the Regulation on the Processing and Protection of the Privacy of Personal Health Data, and other related regulations; Protecting public health, preventive medicine, medical diagnosis, treatment, and care services; Planning and management of health services and their financing; Notifying you about your appointment if you make an appointment; Planning and managing the internal operations of our clinic; Monitoring and preventing abuse and unauthorized transactions; Carrying out risk management and quality improvement activities; Conducting research; Billing for services provided; Verifying your identity; Confirming your relationship with institutions we have agreements with; Sharing requested information with private insurance companies within the scope of financing health services; Responding to any questions or complaints about our health services; Taking all necessary technical and administrative measures for the data security of our clinic systems and applications; Analyzing your use of health services and storing your health data to improve and develop the health services we offer; Retaining information about your health data that must be kept as required by relevant legislation; Ensuring financial reconciliation regarding health services provided to you with institutions we have agreements with; Measuring patient satisfaction, among similar purposes.

2. PERSONS AND INSTITUTIONS TO WHICH YOUR PERSONAL DATA CAN BE TRANSFERRED

Your personal data can be transferred for the purposes stated above and within the framework of the law and related legislation, to: Hekimoğlu Dental Clinic, Social Security Institution, Ministry of Health and its sub-units, Family Health Centers, Private insurance companies (health, retirement, and life insurance, etc.), General Directorate of Security and similar law enforcement agencies, General Directorate of Population and Citizenship Affairs, Turkish Pharmacists Association, Courts, Laboratories, centers, and similar third parties we cooperate with for medical diagnosis, Your authorized representatives, Health institutions to which the patient is referred or applies by themselves, Lawyers, tax consultants, and auditors, including third parties from whom we receive consultancy services, Regulatory and supervisory institutions, official authorities, Suppliers, support service providers, business partners, and other third parties we cooperate with or from whom we receive services, Third parties specified in Articles 8 and 9 of the Personal Data Protection Law.

3. METHOD AND LEGAL REASON FOR COLLECTING PERSONAL DATA

Your personal data is collected in any verbal, written, or electronic environment for the purposes stated above and to provide health services within the legal framework and to fulfill the contractual and legal obligations of Hekimoğlu Dental Clinic in a complete and proper manner.

4. YOUR RIGHTS REGARDING THE PROTECTION OF YOUR PERSONAL DATA

Regarding your processed personal data, you have the following rights under the legislation: To learn whether personal data is processed, To request information if personal data has been processed, To access your personal health data and request them, To learn the purpose of processing personal data and whether they are used in accordance with their purpose, To know the third parties to whom personal data is transferred at home or abroad, To request the correction of personal data if it is incomplete or incorrectly processed, To request the deletion or destruction of personal data, To request that third parties to whom personal data is transferred be notified of the correction, deletion, or destruction of personal data, To object to the occurrence of a result against the person by analyzing the processed data exclusively through automated systems, To request the compensation of damages in case of damage due to the unlawful processing of personal data.
